Please log in to the Certific platform or create your account. If you have not yet created your account, see here for step-by-step instructions.
Once logged in, start by selecting "Start new query"
Select "The query is for myself". If you need to submit a request on behalf of a family member, for example your child or elderly relative, click here to see how.
Choose your clinic using the drop-down arrows and click "Continue"
Start your query by choosing the option that best fits your health concern. If you do not find a suitable option, please choose "Another problem" where you will have a free text box enabling you to describe your health concern.
Firstly, you will be shown some important information such as emergency contact details, how long the query will take and for which symptoms this query is used for.
You will see a progress bar at the top of the page (1) and will always have the option to go back (2) should you need to amend your answer.
Complete the health questionnaire
At the end of the health questionnaire you will have the option to apply for a sick note or carers leave and also have the option to upload photos or documents.

You will now see that your query has been submitted and will see the status of your query, including all previous queries submitted to your medical centre. It is not necessary to remain connected to the Certific platform, you will receive an email notification once your medical centre has responded to your query. No medical information will be included in the notification email, you will need to log back in to Certific to see the response from your medical centre.
